The Arc of Monroe County Medicaid Service Coordination program provides a personalized one-on-one approach, enabling individuals with intellectual and/or other developmental disabilities to achieve self-fulfillment and success. The Arc's Service Coordination department focuses on building long relationships between the Medicaid Service Coordinator, the individuals and their families we serve.
Eligible individuals choose a Medicaid Service Coordinator, whose role is to research available options and resources. The Arc prides themselves in focusing on doing what is best for the individual. The Service Coordinator helps individuals and their families make educated choices about housing, employment, family cash reimbursement, day habilitation, community habilitation, guardianship, Special Sitters, self advocacy and recreational activities, allowing persons with intellectual and other developmental disabilities to live as independently as possible. Some of the options available include Person Centered Planning and Informed Choice.

Like other local families, the Rapkes draw on multiple community supports to get the appropriate assistance for their son and daughter. Taylor and Emily have an Arc service coordinator, Karen, who Beth describes as “amazing; just fantastic.” Karen connects the family to services in the community, such as special educational training for family members.
Highly trained and culturally sensitive, Arc of Monroe Service Coordinators ensure that the various services work together to meet the needs of the service recipient.
The Arc currently serves over 500 individuals that live in residential programs, independently in the community or with family members. Many of the Medicaid Service Coordinators have been working for The Arc for over 5 years. Building long lasting relationships is the key to our success.
