MEDICAID SERVICE COORDINATION


The Arc of Monroe offers Service Coordination to anyone in the community with an intellectual or developmental disability (I/DD). We help you navigate the complex state and local service system so you can make informed decisions for yourself or loved ones. The Arc’s Service Coordination supports more than 600 people, including children and teens, and can make identifying and applying for services easy and convenient.

Linking and referring to services

Maintaining health and safety

Health care

Housing

Medical Liaison

Provides additional support with coordinating, transporting and accompanying people to medical appointments. The medical liaison also assists in facilitating communication between people and their medical team. This offering is unique, and only provided through the Arc of Monroe County.
