Psychiatric Services
The Arc of Monroe provides psychiatric care for medication management by Dr. Susan Chlebowski, New York State Licensed Psychiatrist and Psychiatric Nurse, Maureen Kosmicki, RN.
Provides a Range of Services Including:
- Consultation and Evaluation for Established Patients
- Subsequent Medication Review Visits
- Second Opinion, Consultations Regarding Plan of Treatment and Medication Review
- Collateral/Family Support and Counseling
- Prescribing, Monitoring and Evaluating Effectiveness of Psychoactive Medications
- Consultations regarding approaches in coordination with medication management
Eligibility
- Arc Intake and enrollment process
- Active Medicaid
- Must be OPWDD Eligible
